- Employment Type: Travel Contract – Travel Physical Therapist / Home Health PT
- Contract Duration: 17 weeks (April 5, 2026 – July 31, 2026)
- Guaranteed Hours: 40 hours per week
- Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM (8-hour shifts)
- On-Call/Weekend Requirement: 1 weekend per month, 1 weekday per week (on-call rotation)
- Mileage Reimbursement: Yes, reimbursed between visits (first patient to last patient)
- Coverage Areas: Lexington, KY (zip 40503) and surrounding counties: Madison, Montgomery, Clark, Powell
- Number of Positions: 2 Travel Physical Therapist positions
Qualifications / Job Requirements
- Minimum 1 year of experience as a Physical Therapist (PT) – home health experience highly desirable
- Associate's Degree in Physical Therapy or higher (PT / Physical Therapist)
- Professional certification required: BLS (Basic Life Support)
- Experience with OASIS and HCHB/Point Care documentation systems
- Ability to work independently, manage a home health caseload, and travel between patient homes across multiple counties
- Professional appearance: black, gray, or blue scrubs
Primary Responsibilities – Home Health Physical Therapist
- Perform Start of Care (SOC), evaluations, recertifications, Resumption of Care (ROC), and agency discharges within scope of practice
- Provide high-quality rehabilitation and home health physical therapy services focused on improving patient function and safety
- Maintain accurate, timely documentation in OASIS and HCHB/Point Care to meet clinical and regulatory requirements
- Collaborate with interdisciplinary team members (nurses, OT, SLP, case managers) to optimize patient outcomes
- Meet productivity expectations (30 points per week) and participate in on-call and weekend coverage as scheduled
