• Role: Licensed Physical Therapist (PT) – Home Health / Travel Physical Therapist
• Location: Westborough, MA 01581 (serving Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, Framingham)
• Employment Type: Travel Contract (Home Health)
• Duration: 13 weeks (April 20, 2026 – July 19, 2026)
• Hours: 40 hours/week (32 hours guaranteed)
• Shift: 8:00 am – 4:30 pm (8-hour shifts)

Compensation & Benefits

• Weekly Estimated Pay: $2,030 – $2,190
• Mileage Reimbursement: Reimbursed between patient visits
• Schedule: Flexible scheduling in a travel PT role; occasional changes possible, no regular on-call/weekends required
• Dress Code: Casual professional or scrubs

Job Requirements / Qualifications

• Active Physical Therapy License (Massachusetts)
• Associate's Degree or higher in Physical Therapy (per current job requirements)
• Minimum 1 year clinical experience; homecare experience preferred
• OASIS documentation experience required
• Strong interpersonal and communication skills for patient and family education

Primary Responsibilities

• Provide high-quality home health physical therapy visits across the coverage area, delivering patient-centered care
• Perform comprehensive assessments and develop individualized care plans to meet rehab goals
• Document accurately using OASIS and ensure compliance with home health regulations
• Meet productivity expectations: 26 points in 32 hours or 33 points in 40 hours
• Collaborate with interdisciplinary teams (nursing, social work, case management) to optimize patient outcomes
• Educate patients and families on therapeutic techniques, home exercise programs, fall prevention, and safe mobility
• Maintain professional standards and adhere to regulatory and agency policies
