Job Details

  • Weekly Estimated Pay: $2,030 - $2,190
  • Location: Grants Pass, OR (Josephine County) — Travel Physical Therapy position
  • Setting: Home Health & Hospice (Home Health Physical Therapist / Hospice PT)
  • Contract Duration: 13 weeks (May 1, 2026 – August 1, 2026)
  • Guaranteed Hours: 40 hours/week
  • Shift: Monday–Friday, 8:00 AM – 4:30 PM (8-hour shifts)
  • On Call/Weekend Requirement: None
  • Dress Code: Business casual or any color of scrubs
  • Mileage Reimbursement: Yes, between patient visits

Requirements

  • Doctor of Physical Therapy (DPT) required
  • Current, unrestricted Physical Therapist license for Oregon or ability to obtain licensure prior to start
  • Minimum 1 year of clinical physical therapy experience
  • Home Health experience preferred (Home Health Physical Therapist or Hospice PT experience a plus)
  • Experience with HomeCare Homebase EMR is a plus
  • Strong documentation skills and ability to meet productivity expectations

Responsibilities

  • Provide home-based PT care including Start of Care (SOC), comprehensive evaluations, recertifications, and supervisory visits
  • Deliver rehabilitative and palliative interventions for patients in home health and hospice settings across Josephine County
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams
  • Maintain accurate, timely documentation in EMR systems (experience with HomeCare Homebase preferred)
  • Meet productivity expectations (30 points per week) while providing compassionate, patient-centered care
