Opportunity:

Home Health PT - West Portland OR and Surrounding Areas

Job Details:

• 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
• May visit up to 6 patients per day with 30 productivity points standard
• No weekend requirements
• No on-call requirements
• 1 year of skilled home health experience required
• First-time travelers accepted
• Local candidates accepted within a 50-75 mile radius
• Facility may call off up to one shift per pay period (every two weeks)
• Traveler must have their own reliable transportation
• Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system does a good job grouping clients to minimize drive and windshield time
• OASIS experience required
• Charting system: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
• Attire: Business casual; no jeans; scrubs allowed but not required with no specific color requirement
• Mileage reimbursement provided at IRS standard rate
• Parking is free
• Patient population includes adults and geriatrics with diagnoses such as Dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke
• Required certifications: BLS
• Preferred certifications: Wound Care
• Required skills include lifting and transfer, ordering supplies, and scheduling appointments
• Preferred skills include billing, home health, and OASIS documentation
• Epic charting experience preferred
