Description

Travel RN Critical Care Position

Location: Cleveland, Ohio
Facility: Cleveland Medical Center
Hospital System: University Hospitals
Contract Length: 12 months


Required Credentials and Certifications

  • BLS (Basic Life Support)

    • American Heart Association only
    • Required for clinical positions
  • ACLS (Advanced Cardiovascular Life Support)

    • American Heart Association only
  • NIHSS (National Institutes of Health Stroke Scale)

  • License/Certificate Verification

    • Valid permanent RN license required (temporary licenses not accepted)
    • Primary source verification within 30 days of start, updated upon expiration

Health and Immunization Requirements

  • COVID-19 Vaccine

    • Preferred but not required; proof of vaccine series or approved exemption
  • MMR (Measles, Mumps, Rubella)

    • Positive titers within last 15 years required
    • Booster protocol applies if low titer
  • Varicella

    • Positive titers within last 15 years required
    • Booster protocol applies if low titer
  • Hepatitis B

    • Proof of full series, partial series with declination, or positive titers within 15 years
  • Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination within past 10 years required; no declinations accepted
  • Influenza Vaccine

    • Required during flu season (Oct-Mar) or approved medical/religious exemption
  • 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Negative test within last 6 months required
    • If positive, clear chest x-ray and current TB questionnaire required
  • Positive PPD

    • Negative chest x-ray within 5 years and annual TB questionnaire
  • Fit Test

    • Completed within 1 year of start; specific mask types accepted
  • Physical/Health Statement

    • Pre-employment physical exam within 1 year stating ability to work without restrictions

Professional and Employment Verification

  • UHS I-9 Attestation

    • Proof of eligibility to work in the U.S.
  • UHS Ohio Fingerprints

    • Ohio BCI&I check and FCRA-compliant national criminal background check required
    • Attestation allowed if fingerprints pending, with documentation
  • UHS Criminal Background Check Results

    • Completed within 6 months prior to start
    • Includes SSN trace, county searches, national criminal sweep, and NSO
  • UHS Social Security Trace

    • Required if not included in background check
  • UHS NSO (National Sex Offender) Check

    • Required if not included in background check
  • UHS National Database Criminal Background Check

    • Required for Ohio residents to meet fingerprint requirements
  • UHS OIG Search

    • Office of Inspector General exclusion list check within 6 months
  • UHS SAM/EPLS/GSA/OFAC

    • Federal exclusion lists check within 6 months
  • UHS Education Verification

    • Highest level of education verified; international equivalency required if applicable
  • UHS Employment Verification

    • Verification of employment within last 5 years through third-party

Testing and Competency

  • UHS Competency Exam

    • Department-specific exam with minimum 80% score, completed within 1 year
  • UHS Core Competencies

    • Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required
  • UHS Skills Checklist

    • Clinical skills checklist completed within 1 year prior to start (initial hire only)
  • UHS Drug Screen

    • 10-panel drug screen within 60 days of start; instant drug screens not accepted

Additional Requirements

  • UHS Submission Packet

    • Resume including education and work history, 2 professional references within last 2 years, and completed job application
  • UHS Badge Photo

    • Headshot photo with plain background, shoulders up, JPEG format
  • Driver's License/State ID

    • Non-expired photo ID required
  • UHS Module Completion Certificate

    • Completion of safety and compliance training modules prior to clearance
