Job Opportunity: Physical Therapist (PT)

Contract Length: 12 weeks
Schedule:

• 40 hours per week
• Shift options: 8 AM - 4 PM or 9 AM - 5 PM
• Weekends as needed
• Epic experience preferred but not required

Key Requirements and Onboarding Credentials

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
• License/Certificate Verification: Must be current within 30 days of start date. State verification required.

Health & Immunizations

• Fit Test: Required prior to start.
• Trinity 10 Panel Drug Screen: Must be completed and resulted before start, including specific substances (no THC).
• COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption accepted (except CA or IL).
• MMR: Proof of 2 vaccinations or IgG titer required.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Optional but recommended; proof or declination accepted (except CA or IL).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray per regional guidelines.
• Varicella: Proof of vaccination or IgG titer required.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap: Current, non-expired vaccine required; declination accepted if no pediatric interaction.

Professional & Compliance Documents

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard employment eligibility verification completed by agency.
• Confidentiality & Security Agreement: Signed agreement on patient and proprietary information required.
• Integrity and Audit Services Form: Signed shortly before start date.
• Non-Employee Upload Form: Completed and uploaded.
• Specialty Specific Competency Testing: Specialty exam completed within the last year required.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma verified by third party screening.
• EPLS (Excluded Parties List System): Report dated within 30 days required.
• GSA/OIG Exclusion Check: Report dated within 30 days required.
• Joint Commission CORE Exam: Completed within the last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, and abuse registry checks required within 30 days prior to start.
• OFAC Sanctions Check: Must be completed within 30 days of start.
