Required Credentials and Onboarding Documentation

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must upload front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required.

Health & Immunizations

• Fit Test: Required prior to start.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances (NO THC) and have MRO signature.
• COVID-19 Vaccine: Proof of September 2024 booster, declination, or exemption (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Optional but recommended; proof or declination accepted (declination not accepted in CA or IL).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year (specific states) or within 5 years elsewhere.
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction.

Professional & Compliance Forms

• Trinity Code of Conduct Form: Signed page 27 required.
• Trinity I9: Standard form completed by agency.
• Confidentiality & Security Agreement: Signed page 5 required.
• Integrity and Audit Services Form: Signed shortly before start.
• Non Employee Upload Form: Pages 1-3 and 28 must be completed and uploaded.
• Conflict of Interest Disclosure Form: Signed page 1 required.
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification; must be dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by third party screening.
• EPLS (Excluded Parties List System): Report dated within past 30 days required.
• GSA/OIG Exclusion Check: Report dated within past 30 days required.
• NSO (National Sex Offender) Check: Dated within 30 days of start; includes all aliases.
• System Access Form: Must be completed and uploaded one week before start.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Check: Must be completed within 30 days of start.

Competency & Skills Testing

• Trinity Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Trinity Joint Commission Testing: CORE exam within the last year.
• Trinity Skills Checklist: Provided by agency for clinical contractors.

Submission Requirements

• Trinity Profile: Resume, skills checklist (within a year), and 2 professional references required for submission.
