• Patient population includes cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson’s, ostomy, catheters, MVA, etc.
• Age range of patients: 30-100 years

Experience & Certification Requirements

• Minimum 3 years clinical experience in home health
• Prior travel experience required (at least 2 previous travel assignments preferred)
• Licensed Physical Therapist (PT) required
• Certifications required: BLS, OASIS experience
• OASIS certification preferred

Clinical & Job Details

• Average caseload: 18-22 clients for Case Manager; PT expected to see 5-6 clients per day
• Mileage typically limited to a 30-mile radius unless emergent need
• No floating or charge nurse responsibilities
• Shift hours: 8:00 AM – 5:00 PM
• Weekend requirements: approximately 1 weekend per month (Saturday/Sunday) with 2 compensatory days during the week
• Holiday work: flexible, as needed
• No call requirements
• No 10-hour shifts; block schedules not allowed
• Self-scheduling allowed within department guidelines
• Documentation system: Netsmart (training provided during orientation)
• Medication system: Not specified
• Orientation: 2-3 days, bi-weekly on Mondays

Clinical Skills & Procedures

• Strong clinical and assessment skills required
• Must be fluent in navigating different EMRs
• Unit-specific procedures include:

  • Phlebotomy
  • Catheters
  • IV therapy
  • Coagulation monitoring (coag-u-check)
  • Port access
  • Wound vacs
  • TPN (Total Parenteral Nutrition)
  • Feeding tubes
  • Drains
  • Nebulizer use

Equipment Provided

• Laptop and iPhone
• Clinical supplies including trunk bin/visit bag with vital signs equipment, wound care supplies, catheters, and backup items shipped to client’s home

Additional Notes

• Respiratory and anesthesia support not available 24/7
• Reliable personal or rental vehicle strongly recommended due to lack of public transportation
• Vehicles should have all-season or snow tires from November through April
• Must provide complete work history including bed count, EMR, and facility type
• Candidates must explain any employment gaps over 90 days

This position is ideal for experienced home health PTs who are flexible, have strong clinical skills, and are comfortable working independently in a travel assignment setting.
