Key Requirements and Credentials

Licenses & Certifications

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back copies with expiration date.
• Valid Nursing License: License/Certificate verification required within 30 days of start. For Iowa, use state verification systems.
• Trinity Child and Adult Dependent Abuse Training: Required for Iowa only.
• NIH Stroke Scale (NIHSS) Certification
• Trinity Specialty Specific Competency Testing: Specialty exam must be completed within the past year.
• Trinity Skills Checklist: Provided by agency for clinical contractors.

Background & Compliance Checks

• Criminal Background Check: Federal, nationwide, county criminal checks from the last 7 years, including SSN verification, dated within 30 days of start.
• Trinity IA Sing Report: Iowa’s Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start.
• Drug Screen: 9 Panel drug screen completed within 30 days of start (no THC).
• Trinity EPLS (Excluded Parties List System) Check
• Trinity GSA/OIG (Office of Inspector General) Check
• Trinity NSO (National Sex Offender) Check
• Trinity OFAC (Office of Foreign Assets Control) Screening: Must be completed within 30 days of start.

Health & Immunizations

• Fit Test
• Physical/Health Statement: Documentation from an exam within the last year certifying fitness to work.
• COVID-19 Vaccine: Proof of vaccine, booster, declination, or exemption required.
• MMR (Measles, Mumps, Rubella): Proof of vaccination or immunity required.
• Hepatitis B: Proof of vaccination series required (titers not accepted).
• Influenza Vaccine: Optional but recommended for the flu season (Oct 1, 2024 - Apr 30, 2025).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Tuberculosis screening required.
• Varicella: Proof of vaccination or immunity required.
• Trinity Specific COVID Questionnaire: Must be completed and uploaded before start.
