Location: Davenport, Iowa
Employment Type: Long-Term Assignment (LTA)

Job Requirements & Details

• Required Experience: NICU experience is mandatory
• Certifications Required:

  • BLS (Basic Life Support)
  • ACLS (Advanced Cardiovascular Life Support)
• Licenses: Valid respiratory therapist license with verification dated within 30 days of start
• Other Requirements:

  • Fit Test
  • Trinity Code of Conduct Form
  • Trinity I9 form
  • Confidentiality and System Security Agreement
  • Integrity and Audit Services Form
  • Non-Employee Upload Form
  • Specialty Specific Competency Testing (must be completed within the last year)
  • COVID-19 Vaccine (proof of booster or approved exemption)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (proof required)
  • Influenza vaccine or approved exemption (valid for current flu season)
  • 2 Step PPD or IGRA TB test (with chest x-ray if positive, per regional requirements)
  • Varicella vaccination or immunity proof
  • Physical/Health Statement confirming fitness to work (within last year)
  • 10 Panel Drug Screen (completed within 30 days of start, no THC)
  • Trinity Profile (resume, skills checklist within a year, 2 professional references)
  • Conflict of Interest Disclosure Form
  • Criminal Background Check (federal, nationwide, county for last 7 years, dated within 30 days)
  • Education Verification (highest academic degree or equivalent)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG exclusion check dated within 30 days
  • Joint Commission CORE Exam (within last year)
  • NSO (National Sex Offender) check dated within 30 days
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)
  • Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Iowa criminal history, sex offender, dependent adult & child abuse registries check)
  • OFAC Sanctions Screening (completed within 30 days of start)

Work Schedule & Other Notes

• Every Other Week (EOW) schedule
• Holiday rotation required

This position requires a highly qualified respiratory therapist with NICU experience and full compliance with all listed certifications, vaccinations, screenings, and documentation prior to start.
