Requirements

• Candidates with an Arizona home address on their driver’s license will NOT be considered (a copy of the driver’s license must be included in submission).
• Minimum 24 months of current ICU experience (required).
• Certifications must be from American Heart Association (AHA) OR American Red Cross, active at time of submission and included in submission (required).
• No license hits (required).
• Must have an active Arizona nursing license at time of submission (verification must be less than 30 days old).
• Travel experience preferred.
• Flu vaccination required; declinations can be applied for (vaccination status must be noted in submission).
• Background requirements are very specific and must be adhered to and attested immediately.
• Return to Office (RTO) requests:

  • Will not be approved within the first week of assignment or on holidays.
  • Cannot exceed 7 days.

Submission Requirements

• Fully completed Banner Cover Page.
• Resume listing all facilities, facility size, bed count, trauma level, and units worked.
• Skills checklist specific to the discipline (must be less than 1 year old).
• Active certifications at time of submission.
• Driver’s license copy included.

Certifications Required

• ACLS (AHA)
• BLS (AHA)

Facility Highlights

• Acute-care facility with a recent $290 million expansion including new surgical suites, a heart and vascular center, and a 200-bed patient tower.
• Level I Adult Trauma Center designation for patients 15 and older, capable of providing comprehensive trauma care.
• Facility includes a new Emergency Department and all-private patient rooms.

Additional Notes

• Learning modules must be completed prior to the start of the assignment.
• Non-billable hours adjusted to 16 hours per assignment.
• Assignments may be canceled with 7 days written notice without penalty.
