Job Opportunity: RN Medical/Surgical

Location: Athens, Georgia
Employment Type: Long-Term Assignment (LTA)

Shift

• 1845-0715 (Evening to early morning shift)

Required Certifications & Licenses

• BLS (American Heart Association) certification required for all direct patient care personnel
• Current nursing license verification dated within 30 days of start
• Must provide scanned copies of certifications and licenses as required

Health & Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (must include specific substances, no THC) completed within 30 days of start
• COVID-19 Vaccine (proof of booster by Sept 2025, or approved declination/exemption except for CA or IL)
• Influenza Vaccine (proof or declination/exemption for 10/1/2024 - 4/30/2025 or 10/1/2025 - 4/30/2026)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series started prior to start date (titers not accepted)
• Varicella (proof of vaccination or IgG titer)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Physical/Health Statement confirming fitness to work (exam within last year)
• Trinity Respirator Evaluation with physician signature

Professional & Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Skills Checklist (provided by agency for clinical contractors)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification) dated within 30 days of start
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• CORE Exam (Joint Commission Testing) within the last year
• NSO (National Sex Offender) check dated within 30 days, including all aliases and locations
• System Access Form completed and uploaded one week before start
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days

Additional Notes

• All documentation must be uploaded to the credentialing system prior to or at onboarding as specified
• Non-clinical contractors are exempt from the Skills Checklist requirement
• Declinations for vaccines are not accepted in California or Illinois

This position requires compliance with extensive credentialing, health, and professional standards to ensure patient safety and regulatory adherence.
