Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)

Required Certifications & Licenses

• BLS: American Heart Association certification required for all direct patient care personnel.
• ACLS: Required for RNs in CCU, ED, PACU, MICU, SICU.
• NIHSS: Required.
• License/Certificate Verification: Must be dated within 30 days of start; state verification required (e.g., NURSYS for RNs).

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed.
• COVID-19 Vaccine: Proof of booster by Sept 2025, declination or exemption accepted except for CA or IL.
• MMR: Proof of 2 vaccinations or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for 10/1/2024–4/30/2025 season; declination not accepted for CA or IL.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing with chest x-ray if positive (within last year for some states, within 5 years for others).
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction.

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Signed form required.
• Integrity and Audit Services Form: Signed shortly before start.
• Non Employee Upload Form: Completed and uploaded (specific pages required).
• Specialty Specific Competency Testing: Copy of specialty exam completed within the last year.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, county checks for last 7 years plus SSN verification; must be dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by third party screening.
• EPLS (Excluded Parties List System): Report dated within past 30 days required.
• GSA/OIG Check: Report dated within past 30 days required.
• Joint Commission Testing: CORE exam within last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start; includes alias and location checks.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registry check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Check: Must be completed within 30 days of start.
