Job Description

• Position: Cath Lab Technician
• Call may be required

Required Credentials and Compliance for Onboarding

Professional Requirements

• Prime CA Sexual Harassment Prevention Training (valid for 2 years)
• Driver's License, State ID, or Passport
• Prime OIG clearance
• Prime Criminal Background Check (within past 60 days; covers past 7 years)
• Prime Agency Attestation (must match credential dates)
• Prime SAM/EPLS exclusion search (date must be visible)
• Prime Form I-9 (Employment eligibility verification) – agency attestation accepted
• Prime Resume (required for submission)
• Prime Reference #1 and #2 (required for submission)
• License/Certificate Verification (Primary Source Verification within 30 days of start)
• Prime Core Competencies Exam (must score 80% or higher)
• Prime Skills Checklist
• Prime Specialty Exam (must score 80% or higher)

Personal Health History and Vaccinations

• COVID-19 Vaccine
• MMR (Proof of 2 vaccines or positive titer showing immunity)
• Fit Test (Annual respirator fit test)
• Hepatitis B (declinations not accepted at some locations)
• Influenza Vaccine (required October 1 - May 31; declinations not accepted at some locations)
• TB Test
• Varicella (Vaccination or titer showing immunity)
• Tdap (declinations not accepted at some locations)
• Positive PPD with Negative Chest X-Ray (optional)
• Physical/Health Statement (annual; must be completed within 60 days of start for some locations)
• Prime Drug Screening (minimum 10-panel test within 60 days of start)
• Prime Color-blind Test (updated annually)

Certifications

• DCI/BVP (Disinfection and Biologic Validation Program)
• BLS (American Heart Association or Red Cross)
• ACLS (American Heart Association or Red Cross)

• Prime Badge Photo (JPEG headshot with plain background)
• Prime COA (Certificate of Authenticity)
