Requirements

• Licensing:

  • Active Arizona RN license required at time of submission
  • No license hits allowed
  • Compact license allowed but must be active and verified within 30 days
• Experience:

  • Minimum 24 months of current Labor & Delivery experience (required)
  • Travel experience preferred
• Certifications:

  • Must have current and active certifications from American Heart Association (AHA) or American Red Cross, included in submission
  • Required certifications include:

    • ACLS (AHA)
    • AFM
    • BLS (AHA)
    • NRP
    • PALS (AHA)
• Vaccinations:

  • Flu vaccination required; declinations accepted with proper forms
  • Vaccination status must be noted in submission
• Background:

  • Must meet specific background compliance requirements (to be reviewed immediately)

Submission Requirements

• Candidates with an Arizona home address on their driver’s license will not be considered
• Include a copy of driver’s license in submission
• Fully completed Banner Cover Page
• Resume listing all facilities, facility size, bed count, trauma level, and units worked
• Skills checklist specific to the discipline, dated within the last year
• Active license verification (less than 30 days old)
• Certifications active at time of submission
• Flu vaccination status included
• Must be pre-closed and ready to accept the assignment
• List all departments and shifts the candidate is willing to work
• List all locations the candidate is open to working (if “any,” specify states)
• Return to Office (RTO) requests must be less than 7 days, not allowed during first week or holidays

Additional Notes

• Learning modules must be completed prior to assignment start
• Non-billable hours adjusted to 16 hours per assignment
• Assignments may be canceled with 7 days written notice without penalty
• Start date should be on or very soon after the order start date
