Job Details

• Work Schedule:

  • Monday through Thursday, 10-hour shifts
  • Every 3rd weekend required
• Experience Required: Minimum 1 year
• Additional Duties: May need to float to nearby facilities as needed
• Electronic Health Record System: EPIC

Required Credentials and Health Requirements

• Vaccinations and Immunizations:

  • Proof of two MMR vaccines or positive IGG titer (no declinations accepted; if negative, two MMR doses four weeks apart)
  • Hepatitis B vaccination series completion or signed declination
  • Influenza vaccine during flu season (Oct 1 - Mar 31); exemptions accepted but mask required
  • Documented proof of two Varicella vaccines, immunity evidence, or history verified by healthcare provider (no declinations accepted; if negative, two doses four weeks apart)
  • Tdap vaccine valid within 10 years (no declinations accepted)

• Tuberculosis Screening:

  • Positive IGRA or PPD results required
  • Chest X-ray within one year showing no infection
  • TB Questionnaire completed and updated annually

• Fit Test:

  • Required for all travelers; preferred before start but can be done onsite (PAPR not done onsite)
  • Specific mask types required (3M 1860 Regular, 3M 1860 Small, or 3M 1870+)
  • Updated annually

• Health Statement:

  • Statement of good health or completed physical exam prior to first assignment, updated annually

• Drug Screening:

  • 12-panel drug screen within 30 days prior to start, updated annually
  • Includes testing for amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, and ecstasy
  • Must provide proof of all 12 drugs tested with MRO review if applicable

Professional and Compliance Requirements

• Licensure:

  • Valid driver’s license or state ID matching address on file
  • If working outside home state, address must match home state for compact license

• Background Checks and Verifications:

  • Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and other federal/state checks within 30 days of assignment; updated annually or after break in service > 90 days
  • EPLS (Excluded Parties List System) check within 90 days of start; rerun if break in service > 90 days
  • Employment verification for past 7 years; updated annually or after break in service > 90 days
  • Education verification relevant to clinical work within 90 days of start; rerun if break in service > 90 days
  • At least two professional references prior to first assignment
  • Ascension Skills Checklist completed within one year of start and updated annually
  • Confirmation of assignment documentation on agency letterhead with accurate dates
