Job Opportunity: Registered Respiratory Therapist (Travel)

Job Description

• Position in Respiratory Therapy department
• Facility located in Hoffman Estates, IL

Required Credentials and Certifications

• BLS (Basic Life Support) – Must be AHA certified
• PALS (Pediatric Advanced Life Support) – Must be AHA certified
• ACLS (Advanced Cardiovascular Life Support) – Must be AHA certified
• NRP (Neonatal Resuscitation Program) – Must be AHA/American Academy of Pediatrics certified
• Current Respiratory Therapist License with primary source verification within 30 days prior to assignment start
• Ascension Respiratory Therapy Contract Worker Onboarding Education Attestation (6 hours pre-hire training with signed attestation)
• Ascension Core Competencies – Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score
• Ascension Biometric Consent
• Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation (Illinois)

Health and Immunization Requirements

• MMR (Measles, Mumps, Rubella): Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 – Mar 31); exemptions accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within one year, and completed TB questionnaire; updated annually
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within specified timeframes prior to start
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted

Professional and Compliance Requirements

• License/Certificate Verification: Must be submitted prior to assignment start
• Driver's License/State ID: Must be current and valid, matching address requirements
• Ascension Profile (Resume or Professional Profile)
• At least two professional references completed prior to first assignment
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually
• EPLS (Excluded Parties List System) Check: Run within 90 days of start, rerun if break in service > 90 days
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually
• 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually; includes testing for 12 specified substances with MRO review if applicable
• Education Verification: Verification of relevant education within 90 days of start; updated if break in service > 90 days
• Confirmation of Assignment: Documentation of assignment dates on agency letterhead
• Employment Verification: Third-party verification for past seven years; updated annually or if break in service > 90 days

Additional Notes

• Respiratory Therapy Contract Worker Form is completed onsite by the facility, no need to upload prior to start
• Some onboarding documents and attestations must be uploaded by the agency once completed
• Post-assignment evaluation and legal name change documentation may be required but are optional
