Job Description
- Provide direct patient care in an inpatient rehab setting
- Implement care plans and coordinate discharge needs
- Preferred experience in inpatient rehab nursing

Health & Immunization Requirements
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
- Varicella: Documented proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
- Tdap: Valid within 10 years; no declinations accepted
- Hepatitis B: Documentation of vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted but mask required at all times
- Fit Test: Required annually; preferred prior to start but can be completed onsite
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire
- 2 Step PPD or IGRA within 90 days of start
- Health Statement or physical exam prior to first assignment, updated annually
- 12 Panel Drug Screen within 30 days prior to start, updated annually
Onboarding & Compliance Requirements
- Completion of Ascension onboarding requirements including:
- Nursing Contract Worker Onboarding (onsite)
- Onboarding Education Attestation (pre-hire modules)
- Specialty skills checklist (clinical competency)
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start
- Employment and education verifications within 90 days of start
- Professional references (at least two) prior to first assignment
- Confirmation of assignment documentation on agency letterhead
Additional Notes
- Flu season mask policy applies for October 1, 2024 through March 31, 2025
- Forward facing headshot with white or blue background recommended (optional)
- Post-assignment evaluations and exit surveys may be required
