Job Description

• Med/surg telemetry nurse

Required Specialty Experience

Required Certifications and Licenses

• BLS (American Heart Association only) – Required before onboarding
• ACLS (American Heart Association only) – Required before onboarding
• State Nursing License Verification – Must be completed within 30 days prior to start
• Driver's License or State ID – Valid, current, and unexpired

Required Health and Immunization Documentation

• COVID-19 Vaccine – Optional; upload proof or declination
• MMR – Positive IgG titer or 2 vaccine series required; must show results for Measles, Mumps, and Rubella individually
• Varicella – Positive IgG titer or 2 vaccine series required
• Hepatitis B – Positive antibody titer, 3 vaccine series, or specific declination form required
• Influenza Vaccine – Required or specific declination form; if declined, mask must be worn at all times on assignment
• Tdap – Preferred but not required; within last 10 years or declination form
• TB Test – One of the following required:

  • 2 skin PPDs (1 within 12 months, 1 within 3 months)
  • 1 Quantiferon/Spot test within 12 months
  • For past positives, chest X-ray within 12 months may be required
• Fit Test – Must be completed prior to start and within last 12 months; specific approved mask models required
• Physical/Health Statement – Within last 12 months; must include physician credentials, vitals, height, weight, Snellen eye chart, and statement of no restrictions

Required Professional Documentation and Testing

• Arnot Skills Checklist – Must be fully complete and updated annually for the unit assigned
• Arnot Core Competency Exam – Must score 80% or higher on Core Comp I, II (Nursing), and III
• Agency Background Check – Must be run within 30 days of start; includes SSN address trace, county and national criminal checks, FACIS 3 check
• Agency Drug Screen – Must be run within 30 days of start; specific drug panel required; marijuana testing prohibited by NY state
• Arnot Resume – Must account for all employment and gaps for last 7 years, with education history and dates in MM/YY format
• Professional References – 1 supervisor reference from last 12 months and 1 supervisor or peer reference from last 3 years
• Employment Verification – Verification of most recent employer required (agency counts as employer)
• Job Description Acknowledgement – Signed and dated
• Color Headshot – Straightforward photo with solid background, no filters or sunglasses, JPEG format
• Additional Required Forms – Various onboarding forms including code of conduct, confirmation of assignment, sexual harassment acknowledgement, parking registration, self-study packet and quiz
