Job Opportunity: RN Labor and Delivery
Location: Hoffman Estates, Illinois
Facility: ILARL St Alexius Medical Center
Hospital System: Ascension Health
Unit: 6 East, Labor and Delivery
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Required Credentials and Onboarding Information
Licenses & Professional Credentials
- Nursys Verification: Required within 30 days prior to start. Must disclose any disciplinary actions with FACIS Questionnaire.
- License/Certificate Verification: Current license and primary source verification within 30 days prior to start.
- Driver's License/State ID: Must be current, match address on file, and show front/back.
- Resume/Profile: Required for submission.
- Two Professional References: At least two qualifying references required prior to first assignment.
- Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and others. Must be completed within 30 days of assignment start and updated annually.
- EPLS (Excluded Parties List System): Must be run within 90 days of start and rerun if break in service > 90 days.
- Education Verification: Must be completed within 90 days of start; official transcripts required if verification fails.
- Employment Verification: For the past seven years, updated annually.
Certifications (Must be AHA or equivalent where noted)
- BLS (Basic Life Support): Required prior to onboarding.
- ACLS (Advanced Cardiovascular Life Support): Required prior to onboarding.
- NRP (Neonatal Resuscitation Program): Must be AHA/American Academy of Pediatrics certified, required prior to onboarding.
Testing & Exams
- Ascension Pharmacology Exam: Must be completed prior to start; minimum 80% score; updated every 3 years.
- Ascension Specialty Exam: Completed prior to first assignment; minimum 80% or national benchmark score; updated every 3 years.
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required prior to onboarding.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
Health & Immunizations
- MMR: Documented proof of two vaccines or positive IGG titer. No declinations accepted.
- Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider. No declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Optional; flu season is October 1 through March 31. Declinations accepted with mask requirement.
- Positive PPD / TB Screening: Positive IGRA or PPD results with negative chest X-ray within one year and annual TB questionnaire.
- 2 Step PPD or IGRA: Required prior to start with specific timing guidelines.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite.
- Health Statement: Statement of good health or completed physical exam prior to first assignment; updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually; includes testing for 12 specified substances.
Onboarding Requirements
- Ascension Resume: Required for submission.
- Ascension Onboarding Education Attestation: Completion of a 6-hour pre-hire education module with attestation.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Biometric Consent: Required prior to start.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updated with extensions.
