Job Opportunity: Interventional Radiology Technician
Location: Westerville, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- 4x10 hour shifts, Monday to Friday, 07:00 - 17:30 (varied days)
- 14 days of call including 2 weekends
- Call hours: Monday to Friday 16:00 - 07:00, Saturday and Sunday 24/7

Personal Health History Requirements
- COVID-19 Vaccine: Proof of booster by September 2025, or approved declination/exemption (declination not accepted in CA or IL)
- MMR: Proof of 2 vaccinations or IgG titer lab report
- Fit Test: Required
- Hepatitis B: Must have started vaccination series prior to start; titers not accepted
- Influenza Vaccine: Proof of vaccine or approved declination/exemption for flu season 10/1/2024 - 4/30/2025 (declination not accepted in CA or IL)
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year (specific states) or within 5 years (other regions)
- Varicella: Proof of 1 vaccination or IgG titer lab report
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work
- Drug Screen: 10-panel drug screen completed within 30 days of start (including methadone, phencyclidine, benzodiazepines, cocaine, amphetamines, opiates, barbiturates, and others; NO THC)
Professional Requirements
- Resume and Skills Checklist: Resume, skills checklist within the last year, and 2 professional references required for submission
- Confidentiality Agreement: Signed Trinity Health confidentiality and system security agreement
- Conflict of Interest Disclosure: Signed form required
- Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, dated within 30 days of start
- Education Verification: Highest academic degree or diploma verified by third party screening
- Exclusion Lists: EPLS and OIG exclusion checks dated within 30 days
- Integrity and Audit Services Form: Signed shortly before start
- Joint Commission Testing: CORE exam within the last year
- NSO Check: Nationwide and state-specific checks dated within 30 days
- System Access Form: Must be completed and uploaded one week before start
- Skills Checklist: Provided by agency for clinical contractors
- BLS Certification: American Heart Association BLS certification required, with front and back of card scanned
- License/Certificate Verification: Must be dated within 30 days of start, including online verification for required licenses
- Code of Conduct Form: Signed form on file
- I9 Form: Standard I9 completed by agency
- Specialty Specific Competency Testing: Specialty exam completed within the last year
- Color Vision Test: Required
