Location: Aledo, Iowa

Required Experience

• Trinity 10 Panel Drug Screen (must be completed within 30 days of start; includes testing for multiple substances, NO THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted for CA or IL)
• Influenza Vaccine (proof or declination/exemption with agency letterhead; declination not accepted for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of vaccination or IgG titer)
• Tdap (current, non-expired; declinations accepted if no pediatric interaction)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity Color Vision

Professional & Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality and Information Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed)
• Child and Adult Dependent Abuse Training (Iowa only)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG Exclusion Check dated within 30 days
• Iowa Sing Report (Iowa-specific criminal and abuse registry check within 30 days)
• Joint Commission CORE Exam (within last year)
• OFAC Sanctions Search (within 30 days)
• NSO (National Sex Offender) check dated within 30 days; MI requires additional statewide check
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)

This position is suitable for experienced Medical/Surgical RNs who meet the above credentialing, health, and compliance requirements and are available for a 13-week assignment in Iowa with every other weekend and holiday shifts.
