Required Credentials & Compliance

Candidates must provide proof of the following, with documentation uploaded to the agency system prior to onboarding:

Personal Health History

• COVID-19 Vaccine (including Sept 2025 booster) or approved exemption (declination not accepted for CA or IL)
• MMR vaccination proof
• Fit Test (specific to Iowa mask requirements)
• Hepatitis B vaccination series (titers not accepted)
• Influenza Vaccine (seasonal, with declination/exemption accepted except CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive (within 1 year for MI)
• Varicella vaccination or immunity proof
• Physical/Health Statement confirming fitness to work (within last year)
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)
• Trinity Color Vision test

Drug Screening

• 9 or 10 Panel Drug Screen completed within 30 days of start (NO THC)

Certifications & Licenses

• ARRT Certification (implied by job title and ARRT mention)
• BLS (American Heart Association Basic Life Support) certification required for direct patient care
• License/Certificate Verification dated within 30 days of start (state-specific requirements apply)

Professional & Background Checks

• Resume and Skills Checklist (within 1 year)
• 2 Professional References
• Confidentiality Agreement signed
• Conflict of Interest Disclosure Form signed
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
• Education Verification (highest level of academic education verified)
• EPLS (Excluded Parties List System) check within 30 days
• GSA/OIG Exclusion check within 30 days
• Integrity and Audit Services Form signed prior to start
• Joint Commission Testing (CORE Exam within 1 year)
• NSO (National Sex Offender) check dated within 30 days, including statewide check for MI
• System Access Form completed and uploaded one week before start
• Trinity Skills Checklist (clinical contractors only)
• Code of Conduct Form signed
• I9 form completed by agency
• Specialty Specific Competency Testing completed within 1 year

Additional Notes

• All documentation must be current, valid, and uploaded to the agency system before onboarding.
• Declinations for vaccines are generally not accepted for CA or IL assignments.
• Positive TB tests require a chest x-ray within the last year for Michigan.
• Drug screen panels exclude THC.
• Physical/Health Statement must confirm candidate is fit to work.
