Job Opportunity: Care Coordinator (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 26 weeks (approximately 6 months)

- Required Years of Experience: 1 year in Ambulatory Care (including Cardiac Nursing experience)
- Cardiac Nursing experience specifically with management of Heart Failure and/or Heart Transplant recipients is required.
- Ability to float to other facilities within the system.
Job Description
- Coordinate care for patients with heart transplants, including:
- Immunosuppression monitoring
- Prophylactic antibiotic management
- Monitoring for organ rejection and infection
- Routine patient monitoring
- Preferred experience in managing other organ transplant recipients.
- Experience with Pheonix system and working knowledge of immunosuppressive therapies is a plus.
Licenses & Certifications Required
- California RN License
- AHA Basic Life Support (BLS)
- Advanced Cardiac Life Support (ACLS) - AHA certified, no exceptions
- Other certifications may be required depending on unit assignment (e.g., PALS, NRP, CHEMO, TNCC)
Experience & Skills
- Minimum 1 year of Ambulatory Care experience
- Cardiac nursing experience with heart failure and/or heart transplant patients
- Ability to float to other facilities within the system
Prior Travel Experience
- No explicit requirement for prior travel experience mentioned.
Compliance & Documentation Requirements
- Current and valid Driver’s License or government photo ID matching professional license
- Background checks including 7-year criminal check, EPLS/GSA/SAM, HHS/OIG, Sex Offender Search, Education Verification
- Transcript or diploma acceptable for education verification
- Primary source verification of state license within 30 days of start
- NURSYS verification required for RN license
- Pass/Fail Telemetry and Medication tests required on first day of orientation
- Unit-specific competency exams and core clinical competency exams required annually or within 12 months of start
- Physical exam within 1 year of start, including physician statement of fitness to work
- Fit mask test within 1 year of start (specific mask types required)
- Immunizations and titers required: Tetanus/Diphtheria/Tdap (within 10 years), TB testing (2 negative tests required), Rubeola, Mumps, Rubella, Hepatitis B, Varicella
- Drug testing: Standard 9-panel excluding THC, within 30 days of start and every 2 years thereafter
- Seasonal flu vaccination or facility-specific declination required during flu season
- COVID-19 vaccination and booster requirements, with options for medical or religious exemptions
- Annual TB questionnaire and other health screenings as required
Orientation & Training
- Completion of facility-specific learning modules through the UCSF Learning Management System required ASAP after clearance
- Cyber Security Awareness Training and HIPAA Healthcare Provider Module required
- Pass/Fail Telemetry and Medication tests on first day of orientation
References & Work History
- Two supervisory references required, preferably from most recent employment
- Current and updated resume including last 7 years of work history with explanations for gaps over 30 days
- Verification of 7 years of work history required
Additional Notes
- Ability to float to other facilities within the system is required
- Preferred experience with organ transplant management beyond heart transplants
- Must be able to comply with all facility-specific policies and documentation requirements prior to start
