Job Opportunity: Physical Therapist (PT) - Travel Assignment

Location: Racine, Wisconsin
Employment Type: Travel
Contract Length: 13 weeks


Job Requirements & Details

  • Must hold a current Physical Therapist license.
  • Basic Life Support (BLS) certification required.
  • Minimum 1 year of hospital experience required.
  • Weekend shifts: 6 weekend days per 13-week assignment.
  • No on-call duties.
  • Holidays: rotating schedule with regular staff.
  • May need to float onsite within skill set or to other sites within 40 miles as needed.
  • Experience with EPIC electronic health record system preferred.

Required Credentials & Health Compliance

  • Vaccinations:

    • Proof of two MMR vaccines or positive IGG titer (no declinations accepted). If negative titer, two doses of MMR vaccine four weeks apart required.
    • Documentation of Hepatitis B vaccination series completion or signed declination.
    • Influenza vaccine required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
    • Proof of two Varicella vaccines, serological immunity, or history verified by healthcare provider (no declinations accepted).
    • Tdap vaccine valid within last 10 years (no declinations accepted).
  • Tuberculosis Screening:

    • Positive IGRA or PPD results required, with chest X-ray within one year showing no infection, plus TB questionnaire completed. Annual TB questionnaire updates required.
    • Two-step PPD or IGRA testing within specified timeframes prior to start.
  • Fit Test:

    • Required for all travelers; preferred before start but can be completed onsite. Specific mask types required (3M 1860 Regular, 3M 1860 Small, or 3M 1870+).
  • Health Statement:

    • Statement of good health or completed physical exam prior to first assignment, updated annually.
  • Drug Screening:

    • 12-panel drug screen within 30 days prior to start, updated annually. Must include all specified substances and MRO review if applicable.

Professional Requirements

  • Valid Driver’s License or State ID (must not be expired and meet address requirements).
  • Resume/Profile submission required.
  • At least two professional references required prior to first assignment.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and other checks within 30 days of assignment start; updated annually or after breaks in service.
  • EPLS (Excluded Parties List System) check within 90 days of start; rerun if break in service > 90 days.
  • Skills checklist for specialty to be completed within one year of start and updated annually.
  • Education verification relevant to clinical work within 90 days of start; must be rerun if break in service > 90 days.
  • Confirmation of assignment documentation required upon initial assignment and updates with extensions.
  • Employment verification for past seven years required; updated annually or after breaks in service.
  • Badge photo with white or blue background required (no alterations or filters).

Additional Notes

  • Prior travel experience is NOT explicitly required for this position.
  • Specialty years of experience required: Minimum 1 year hospital experience in Physical Therapy.
