Job Details

• Specialty: Registered Nurse (RN) in Medical/Surgical and Telemetry units
• Schedule: Every third weekend; Holiday rotation every other holiday

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back card images.
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Must provide front and back card images.
• License/Certificate Verification: Must be dated within 30 days of start. State verification required (e.g., NURSYS for RNs).
• NIHSS (National Institutes of Health Stroke Scale): Required.
• TNCC (Trauma Nursing Core Course): Optional but preferred.

Health & Immunization Requirements

• Fit Test: Required.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed.
• COVID-19 Vaccine: Proof of booster by September 2025, declination or exemption accepted except in CA or IL.
• MMR: Proof of 2 vaccinations or IgG titer lab report required.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for 10/1/2024 - 4/30/2025 season. Declination not accepted in CA or IL.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray per regional guidelines.
• Varicella: Proof of 1 vaccination or IgG titer lab report required.
• Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional & Compliance Requirements

• Trinity Profile: Resume, skills checklist (within 1 year), and 2 professional references required for submission.
• Background Checks:

  • Federal, Nationwide, County criminal checks covering last 7 years, including SSN verification.
  • Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• Education Verification: Highest academic degree or diploma/GED verified by third party screening.
• EPLS (Excluded Parties List System) & GSA/OIG Checks: Must be dated within 30 days of start.
• NSO (National Sex Offender) Check: Must be dated within 30 days of start; includes alias and location checks.
• Trinity Code of Conduct, Confidentiality Agreement, Conflict of Interest Disclosure, Integrity and Audit Services Form: All must be signed and on file.
• Trinity System Access Form: Must be completed and uploaded one week before start.
• Trinity Specialty Specific Competency Testing: Specialty exam must be completed within the last year.
• Trinity Joint Commission Testing: CORE exam within the last year required.
• Trinity Skills Checklist: Provided by agency for clinical contractors.
• Trinity Non Employee Upload Form: Must be completed and uploaded.
• Trinity OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.
• Trinity Child and Adult Dependent Abuse Training: Required for Iowa assignments.
