  • Influenza Vaccine: Required or declination (seasonal)

  • Physical/Health Statement: Annual, must state clear to work

• Certifications:

  • BLS (American Heart Association or Red Cross)
  • ACLS (American Heart Association or Red Cross)
  • DCI/BVP (Workplace Violence Prevention training)

• Licenses & Professional Requirements:

  • Valid professional license verification (Primary Source Verification within 30 days of start)
  • Driver’s License, State ID, or Passport
  • Prime OIG check
  • Prime SAM/EPLS exclusion search
  • Prime Criminal Background Check (within 1 year; some states require before start)
  • Prime Agency Attestation (completed after all credentials uploaded)
  • Prime Form I-9 (Employment eligibility verification)
  • Prime Badge Photo (clear headshot, JPEG)

• Testing & Exams:

  • Prime Core Competencies Exam (minimum 80%)
  • Prime Skills Checklist
  • Prime Specialty Exam (minimum 80%)
  • Prime Basic Pharmacology Exam (minimum 80%)
  • Prime Basic Rhythm Interpretation Screening (minimum 80%)
  • Annual Respirator Fit Test (must be completed prior to start if not available onsite)
  • Prime Drug Screening (minimum 10-panel test including THC, within 60 days of start)

• References & Documentation:

  • Updated Resume at time of offer acceptance
  • Two professional references required (one must be from a manager or direct supervisor)
  • Signed Prime Job Description
