Location: Albany, New York
Employment Type: Long-Term Assignment (LTA)

- On-call only 1-2 times per week
- Must be able to arrive within 30 minutes of being called in

- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella (proof of vaccination or IgG titer)
- Physical/Health Statement (exam within last year confirming fitness to work)
- Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)
Professional Requirements (Must be completed before onboarding)
- Resume and Skills Checklist (within the last year)
- Confidentiality and Security Agreement
- Conflict of Interest Disclosure Form
- Criminal Background Check (federal, nationwide, county for last 7 years; dated within 30 days)
- Education Verification (highest academic degree or diploma verified)
- EPLS (Excluded Parties List System) check dated within 30 days
- GSA/OIG Exclusion Check dated within 30 days
- Integrity and Audit Services Form (signed shortly before start)
- Joint Commission CORE Exam (within last year)
- National Sex Offender Registry (NSO) check dated within 30 days
- System Access Form (completed and uploaded one week before start)
- Skills Checklist (agency provided for clinical contractors)
- License/Certificate Verification (dated within 30 days; state verification required)
- BLS Certification (American Heart Association; front and back of card required)
- Code of Conduct Form (signed)
- I9 Form (standard, completed by agency)
- Specialty Specific Competency Testing (specialty exam within last year)
Summary
This is a long-term RN CVOR position based in Albany, NY, requiring on-call availability 1-2 times per week with a rapid response time. Candidates must meet extensive health, licensing, and professional compliance requirements prior to onboarding. Prior travel experience is not required.
