Job Details

• Full-time, 40 hours per week

Required Qualifications

• Valid Physical Therapist (PT) license for Florida
• Basic Life Support (BLS) certification
• Minimum years of experience: Not explicitly stated; assume standard for acute care PT (typically 1+ years)

Health and Safety Requirements

• Documented proof of two MMR vaccines or positive IGG titer (no declinations accepted)

Professional Requirements

• Valid driver’s license or state ID (must not be expired and meet address requirements)
• Resume/profile submission required
• At least two professional references prior to first assignment
• Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment
• EPLS (Excluded Parties List System) check within 90 days of start
• Skills checklist completed within one year of start, updated annually
• Education verification within 90 days of start
• Employment verification for the past seven years
• Confirmation of assignment documentation upon initial assignment and updates with extensions
• Badge photo with white or blue background (no alterations)

Additional Notes

• All vaccinations and health screenings must be up to date and documented as required
• Compliance with all credentialing and onboarding requirements is mandatory prior to start
• No declinations accepted for MMR, Varicella, and Tdap vaccines
