Physical Therapist - New York, Greene

Job Summary

Provide physical therapy evaluations and treatments, ensuring services are administered timely, accurately, and reliably. Maintain clinical documentation and adhere to service delivery standards including customer satisfaction, quality processes, communication, compliance, and environment of care.

Job Responsibilities

  • Assess patient strengths, limitations, and needs using standardized tests.
  • Develop and implement individualized short- and long-term treatment goals.
  • Administer and interpret physical therapy services; adjust treatment plans as needed.
  • Maintain accurate and timely patient treatment records in electronic health systems.
  • Ensure documentation meets reimbursement and regulatory requirements within two business days.
  • Establish effective working relationships with patients, families, and interdisciplinary teams.
  • Participate in patient care conferences and collaborate for coordinated care.
  • Educate patients and families on treatment plans, safety, and self-care techniques.
  • Participate in discharge planning and coordinate referrals, including durable medical equipment.
  • Supervise and direct Physical Therapist Assistants during treatment sessions.
  • Maintain licensure and pursue continuing education to stay current with professional standards.
  • Engage in performance improvement initiatives to enhance quality of care.

Contract Length

  • 13 weeks

Required Qualifications

Education

  • Graduate of an accredited school of physical therapy with a bachelor’s degree or equivalent.

Experience

  • Minimum of 1 year of experience as a Physical Therapist, including experience within the last year.

Licensure & Certifications

  • Valid Physical Therapist license in New York State.
  • BLS Certification issued by the American Heart Association or American Red Cross.

Preferred Qualifications

  • Master’s or Doctorate Degree in Physical Therapy.
