Physical Therapist (PT) - Long Term Care (LTC)

Job Description

• Assess and diagnose patients' mobility and functional deficits.
• Develop individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability.
• Perform patient evaluations including history, systems review, and specific tests/measures.
• Establish and adjust plans of care based on ongoing reassessment and physician orders.
• Initiate and manage discharge planning from the first visit through treatment completion.
• Supervise and delegate tasks to ancillary staff and students in compliance with state laws and policies.
• Complete accurate and timely documentation according to department, accreditation, and insurance standards.
• Communicate effectively with patients, families, caregivers, and healthcare team members using AIDET principles.
• Provide education tailored to the needs of patients and their support systems.
• Participate in professional development and quality improvement activities.
• Maintain a safe environment for patients, visitors, and staff.
• Practice within the scope defined by the Texas Physical Therapy Practice Act.

Required Qualifications

• Valid Physical Therapist license in Texas
• Minimum 2 years of Physical Therapist experience
• Physical Therapist experience within the last year
• Experience in a Long Term Care setting
