Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Summary

Registered Nurse specializing in Critical Care for Cardiac ICU patients.


Required Qualifications

  • Valid RN license in Ohio
  • Minimum 1 year of ICU (Intensive Care Unit) experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification issued by the American Heart Association
  • Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)

Health and Safety Requirements

  • Immunizations and Health Screenings:
    • MMR: Positive titers within last 15 years or appropriate booster protocol.
    • Varicella: Positive titers within last 15 years or appropriate booster protocol.
    • Hepatitis B: Proof of full series, partial series with signed declination, or positive titer within 15 years.
    • Tdap: Vaccination within last 10 years (no declinations accepted).
    • Influenza Vaccine: Preferred during flu season (Oct-Mar); declination requires mask-wearing and completion of specific forms.
  • Tuberculosis Testing:
    • 2-Step PPD or IGRA (QuantiFERON/TSPOT) within last 6 months, annual requirement. If positive, clear chest X-ray (valid for 5 years) and current TB questionnaire required.
  • Fit Test: Annual respirator fit test completed within 1 year prior to start (specific approved mask models required).
  • Physical Exam: Pre-employment physical exam within 1 year prior to start, confirming ability to work without restrictions.
  • Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted. New screen required if break in assignment exceeds 60 days.

Professional and Compliance Requirements

  • Background Checks:
    • Criminal background check within 6 months prior to start, including Social Security trace, county searches (last 7 years), national criminal sweep, and sex offender registry.
    • Ohio fingerprinting requirements based on residency duration:
      • ≥5 years: Ohio BCI&I check and FCRA-compliant national database check.
      • <5 years: Ohio BCI&I check and FBI fingerprint check (FCRA-compliant).
    • OIG exclusion check and SAM/EPLS/GSA/OFAC screening within 6 months prior to start.
  • Employment and Education Verification:
    • Verification of highest education level through third party (international equivalency required if applicable).
    • Employment verification for last 5 years through third party (excluding agency employment).
  • I-9 Verification: Completed and on file confirming eligibility to work.
  • Skills and Competency:
    • Skills checklist specific to clinical area completed within 1 year prior to start (one-time requirement).
    • Competency exam with minimum 80% score within 1 year prior to start (one-time requirement).
    • Core competencies (HIPAA, Joint Commission NPSG, OSHA topics) with minimum 80% score within 1 year prior to start (one-time requirement).
  • Photo ID: Non-expired government-issued photo ID required.
  • Badge Photo: Headshot with plain background, shoulders up, JPEG format.
  • Compliance Training: Completion of required safety and compliance training modules prior to clearance; annual renewal required.
