Physical Therapist - Home Health (Travel/Contract)
Location: Augusta, Georgia
Contract Length: 12 weeks
Job Summary
- Provide physical therapy services in a home health setting for adult to geriatric patients.
- Patient ratio approximately 1:5.
- Work within a 4-county travel area.
- Floating within facility/scope of practice as needed.
- No weekend shifts required.
- Expected to work major holidays if they fall within the assignment period (up to two holidays).
- Shift: Day shift, 8:00 AM to 4:30 PM (5 days a week, 8 hours per day).
Required Qualifications
- Valid Physical Therapist license in Georgia.
- Minimum 1 year of Physical Therapist experience within the last year.
- Current Basic Life Support (BLS) certification (AHA or ARC accepted).
Compliance & Screening
- Full compliance with all onboarding requirements is mandatory to start on time; delays may result in a 2-week pushback due to orientation scheduling.
- Clinical screening will be conducted to determine clinical fit before final approval.
- Background check must be completed within 30 days prior to start, including education verification and criminal history checks.
- Drug screening: Negative 10-panel drug test within 60 days of start, no pending results allowed.
- Physical exam required within 12 months of start.
- Annual TB questionnaire and negative TB test or chest x-ray within 12 months required.
- Immunizations/vaccinations required or proof of immunity: MMR (Measles, Mumps, Rubella), Varicella, Hepatitis B (or signed declination), and seasonal flu vaccine or declination.
- Color blindness testing required.
- Skills checklist and work history verification required, with some documents allowed pending start with proof of processing.
- Two clinical references required.
Additional Notes
- No specific call requirements.
- Scrub color is flexible (any color allowed).
- Orientation hours vary based on contract length.
- Must review and sign facility-specific policies including Code of Conduct and Dress/Professional Appearance Policy prior to start.
- Candidate photo required for badge issuance.
