Job Opportunity: Care Coordinator (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
Contract Length: Approximately 26 weeks
Job Description
- Coordinate care for patients with heart transplants, including:
- Immunosuppression monitoring
- Prophylactic antibiotic management
- Monitoring for organ rejection and infection
- Routine patient monitoring
- Manage cardiac nursing care, specifically for heart failure and/or heart transplant recipients
- Ability to float to other facilities within the system
Required Qualifications
- Valid Registered Nurse (RN) license in California
- Minimum 1 year of Care Manager experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- American Heart Association (AHA) Basic Life Support (BLS) certification
- Ambulatory care experience
- Transplant experience
Preferred Qualifications
- Experience managing other organ transplant recipients
- Experience with Phoenix electronic health record system
- Working knowledge of immunosuppressive therapies
Required Certifications & Competencies
- AHA BLS (Basic Life Support)
- Advanced Cardiac Life Support (ACLS) - AHA certified (required for ICU and telemetry units)
- Medication Test/Pharmacology Competency Exam (passing score ≥ 80%) within 1 year prior to start
- Unit-specific competency exam (passing score ≥ 80%) within 12 months of start
- Core clinical competency modules (Parts I, II, III) with passing scores (≥ 80%) completed within 1 year prior to start and updated annually
- Pass/Fail Telemetry Exam on first day of orientation (for nurses working in telemetry and critical care areas)
Background & Verification Requirements
- 7-year criminal background check including specific counties and nationwide databases, completed within 30 days of start and renewed annually
- Education verification or copy of diploma/transcript
- Primary source license verification within 30 days of start
- Employment verification covering 7 years of work history
- Two supervisory references from recent employment
- Skills checklist updated annually
Health & Immunization Requirements
- Physical exam within 1 year of start, including physician statement of fitness to work
- Fit mask test within 1 year of start (specific approved mask types required), updated annually
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declinations not accepted except medical exemptions)
- Two negative TB tests (PPD, QuantiFERON, or T-spot) with specific timing requirements, or chest X-ray if positive TB history
- Annual TB questionnaire within 30 days of start
- Immunity or vaccination documentation for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, series, or declination)
- Varicella (chickenpox)
- Standard 9-panel drug test (excluding THC) within 30 days of start, re-run every 2 years
- Seasonal flu vaccination or facility-specific declination for current flu season (Nov 1 – Apr 30)
- COVID-19 vaccination is strongly recommended but not required
Facility-Specific Requirements
- Completion of facility-specific orientation modules and training prior to or on Day 1
- Review and initial facility policies including COVID-19 preparedness, parking, uniform guidelines, and cybersecurity awareness
- Completion of emergency contact information form
- Compliance with facility’s pass/fail testing policies (medication and telemetry exams)
- Completion of any required background and health documentation uploads prior to start

- This role involves working exclusively day shifts (8 hours)
