Physical Therapist - Home Health (Travel/Contract)

Location: Augusta, Georgia
Contract Length: 12 weeks


Job Summary

  • Provide home health physical therapy services to adult and geriatric patients.
  • Focus areas include assessments, documentation, orthopedics, and neurology.
  • Average daily caseload: approximately 6 visits per day, with an average census of 80 patients.
  • Work setting requires use of Homecare Homebase (HCHB) electronic documentation system.
  • Shift: Day shift, 5 days per week, 8 hours per day (8:00 AM – 4:30 PM).
  • Floating within facility/scope of practice as needed.
  • No weekend shifts required.
  • Must be available to work major holidays if they fall within the assignment period (expectation varies depending on number of holidays).
  • Return to Office (RTO) requirement: maximum 7 days.

Required Qualifications

  • Valid Physical Therapist license in Georgia.
  • Minimum of 1 year of physical therapy experience, including experience within the last year.
  • Current Basic Life Support (BLS) certification.
  • Experience in a Home Health setting.

Compliance & Screening

  • Full compliance with credentialing and onboarding required to start on time; delays may result in a 2-week pushback due to orientation scheduling.
  • Clinical screening will be conducted to determine clinical fit before final approval.
  • Background check must be completed within 30 days prior to start, including education verification and criminal history checks.
  • Drug screening: Negative 10-panel drug screen within 60 days of start (instant drug screen acceptable).
  • Physical exam required within 12 months prior to start.
  • Annual TB questionnaire and negative TB test (PPD, QGold, or TSpot) within 12 months required. Candidates with prior positive TB test must provide additional documentation including a negative chest x-ray within 6 months of start.
  • Immunizations/vaccinations required or documentation thereof:
    • MMR (Measles, Mumps, Rubella) – 2 vaccines or positive titer with boosters if needed.
    • Varicella – 2 vaccines or positive titer with boosters if needed.
    • Hepatitis B surface antibody titer or signed declination.
    • Flu vaccine or signed declination (required during flu season).
    • TDAP highly recommended but not required.
  • Color blindness testing required; alternative assessment available for blood collection tasks if needed.

Documentation & Other Requirements

  • Valid driver’s license or state ID photo required.
  • Clear, passport-style photo with solid background for badge.
  • Proof of current automobile liability insurance required.
  • Review and sign facility’s Code of Conduct and Dress/Professional Appearance Policy prior to start.
  • Skills checklist updated within the last 12 months required.
  • Verification of most recent work history required; can be pending at start with proof it is in process.
  • Two clinical references required.

Additional Notes

  • No call requirements specified.
  • Scrub color is flexible (any color allowed).
  • Number of non-billable orientation hours depends on contract length (e.g., 8 hours for 8-week assignment).
  • Locals accepted at the same rate.
