Location: Wichita, Kansas
Employment Type: Long-Term Assignment (13 weeks)
Shift: 08:00 AM – 04:30 PM
Job Description
- Work with Electronic Health Record system: Cerner OneChart
- Dress code: Black scrubs
Required Qualifications
- Valid Physical Therapy Assistant license in Kansas
- Minimum of 1 year of Physical Therapy Assistant experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association
Required Credentials and Documentation
Submission Requirements (Before Consideration)
- Professional resume/profile
- Two professional references
- Valid driver’s license or state ID (must not be expired and address must comply with compact license rules)
- BLS certification
Onboarding Requirements (Must be completed prior to or at start)
- Signed assignment agreement (offer letter, initial confirmation, and extension)
- Health documentation including:
- MMR: Proof of two vaccines or positive immunity titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or verified history (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Hepatitis B: Completion of vaccination series or signed declination
- Influenza vaccine (optional; if declined during flu season, mask required)
- Tuberculosis screening: 2-step PPD or IGRA test with annual TB questionnaire; if positive, chest X-ray within 1 year required
- Fit test for respiratory protection (preferred before start; onsite completion possible)
- Health statement or physical exam confirming good health, updated annually
- Background checks and verifications:
- Criminal background check including SSN trace, OIG search, national sex offender registry, and others within 30 days of start; updated annually or after >90-day break
- Employment verification for past 7 years; updated annually or after >90-day break
- Education verification relevant to clinical work within 90 days of start; updated after >90-day break
- EPLS (Excluded Parties List System) check within 90 days of start; rerun if break >90 days
- Skills checklist for specialty, completed within one year and updated annually
- 12-panel drug screen within 30 days prior to start; updated annually or after >90-day break
- Badge photo: Forward-facing headshot with white or blue background, no alterations
