Registered Nurse – Medical/Surgical Intensive Care

Location: Prescott, Arizona
Contract Length: 13 weeks
Shift: Nights
Hours: 12-hour shifts, approximately 36 hours per week

Unit Overview

• Critical Care service line includes CVICU (12 beds), COVID unit (8 beds), Medical-Surgical ICU (8 beds), and PCU (16-20 beds with double occupancy).
• Nurse-to-patient ratios:

  • ICU: 1:1 to 1:2 depending on acuity and staffing
  • PCU: 1:3 to 1:4
• Patient population includes COVID (manual proning), low-level neuro, EVD, DKA, sepsis, PN, ARF, CRRT, rhabdomyolysis, post-MI, detox, chest tubes, arterial lines, Swan-Ganz catheters, post-surgical patients, Impella, TAVR, cardiac surgical cases, VAD devices, STEMI, PCI, CEA, IABP.

Required Certifications & Exams

• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support)
• Must pass a dysrhythmia exam with a minimum score of 80% within two attempts prior to contract signing.

Required Skills & Experience

• Strong EKG interpretation skills required.
• Experience with weaning and troubleshooting ventilators.
• Ability to titrate critical care drips including pressors (e.g., levophed), paralytics, sedation (propofol), antiarrhythmics, dopamine, heparin, argatroban.
• Preferred experience with IABP and Prismaflex CRRT.
• Experience caring for fresh open heart surgery patients is a plus if skills can be verified.
• Nurses must be able to troubleshoot ventilators and assist with weaning.
• Must be comfortable working across all critical care areas and float as needed.

Staffing & Support

• Charge nurses may take patient assignments only as a last resort.
• House supervisor coverage 24/7 with some exceptions.
• Intensivists available in-house 24/7.
• Nurse Practitioners provide supplemental coverage.
• Respiratory Therapists cover all areas but are not present in ICU 24/7.
• No Telemetry/Monitor Techs on unit; nurses are responsible for reading their own telemetry strips (Nihon Khoden monitors).

Equipment & Procedures

• Familiarity with ventilators, Glidescopes (used during intubation), BiPAP, CPAP, HFNC, Vapotherm required.
• Ability to manage arterial lines, Swan-Ganz catheters, chest tubes, and other critical care devices.

Scheduling & Work Environment

• 7 pm to 7 am shifts.
• Schedules created 4 weeks in advance with some self-scheduling allowed, subject to manager approval.
• Weekend requirement: 4 weekend shifts per pay period (including Friday/Saturday nights and Saturday/Sunday days).
• Holiday requirement: 1 holiday and 1 holiday eve per contract period; not all holidays require coverage.
• Possible on-call up to once every 6 weeks during low census; no scheduled call.
• Floating required within critical care units and possibly to PCU and MST as needed.
• Dress code: professional scrubs of any color except camouflage; transitioning to navy scrubs in October.

Orientation

• 8 hours general hospital orientation.
• 1 full 12-hour shift with a preceptor on the unit.
• 2nd 12-hour shift with a buddy for support.
• Additional orientation/checkoff for specialized skills such as open heart surgery or CRRT if needed.

Additional Notes

• Radius rule: candidates must not have worked full-time, part-time, or PRN within 50 miles at any affiliated facilities in the past year.
• Prescreen and auto-offer available if return-to-office is less than 7 days and no holiday requests.
• No nurse aides on the unit.
• Nurses must be able to interpret ABGs.
• Charge nurses assist with rapid response and code blue teams.

Required Qualifications Summary

• Registered Nurse license in Arizona.
• Minimum experience: Intensive Care specialty (years not explicitly stated; assume standard ICU RN experience).
• BLS and ACLS certifications current.
• Strong critical care skills including ventilator management, EKG interpretation, and titration of drips.
• Ability to pass dysrhythmia exam prior to contract.
