Certified Occupational Therapy Assistant (COTA) - Travel Assignment

Location: Pampa, Texas
Contract Length: 13 weeks
Shift: Day shift, 08:00 AM – 05:00 PM
Employment Type: Long-term assignment (LTA)


Job Summary

Seeking a Certified Occupational Therapy Assistant (COTA) with 1-2 years of experience to provide rehabilitation services. This is a travel assignment requiring full-time day shift availability.


Required Qualifications

  • Valid Certified Occupational Therapy Assistant license in Texas
  • Minimum 1 year of Certified Occupational Therapy Assistant experience within the last year
  • Basic Life Support (BLS) certification issued by American Heart Association or Red Cross, maintained throughout employment
  • National Board for Certification in Occupational Therapy (NBCOT) certification
  • Completion of workplace violence prevention training (DCI/BVP)

Onboarding and Compliance Requirements

  • Proof of immunizations or immunity for:
    • MMR (2 vaccines or positive titer)
    • Varicella (vaccination or titer)
    • Hepatitis B (3 immunizations, immune titer, or declination accepted)
    • Influenza vaccine or declination (required November 1 – March 31)
    • Tdap (declination only for medical reasons with PCP note)
  • COVID-19 vaccination (2 doses plus booster) or completed declination (optional)
  • Tuberculosis (TB) test within one year of start date
  • Positive PPD requires negative chest X-ray proof (optional)
  • Annual respirator fit test (3M 1860(s) mask or facility-approved alternative)
  • Annual physical/health statement confirming clearance to work (within 60 days of start)
  • Valid Driver’s License, State ID, or Passport
  • National criminal background check within one year of start date (state-specific timing requirements apply)
  • Drug screening within 60 days of start (minimum 10-panel including THC; CA locations require additional saliva THC test)
  • Completion of facility-required competency exams and skills checklists with minimum 80% score
  • Completion of orientation packet including essentials form and post-test
  • Signed current job description (within the year)
  • Education verification (diploma or transcripts if third-party verification unavailable)
  • Prime agency attestation and SAM/EPLS exclusion search completed prior to onboarding
  • Two professional references required, including at least one from a manager or direct supervisor
  • Annual color-blind test
