Description

Physical Therapist (Post Acute) - Travel Contract

Location: Sacramento, California
Contract Length: 13 weeks


Job Overview

  • Provide physical therapy primarily in a skilled nursing facility setting with some float to an inpatient acute care hospital on the same campus.
  • Treat approximately 8-9 patients per day in a post-acute care environment.
  • Patient population includes geriatric patients with chronic conditions such as total joint replacements, stroke (CVA), deconditioning, pneumonia, balance difficulties, COPD, orthopedic and neurological diagnoses.
  • Work closely with an interdisciplinary team.
  • Perform transfers independently and demonstrate excellent communication skills.
  • Use Cerner EMR system.
  • No rehab aides, OTAs, or PTAs available.
  • Orientation specific to floor/unit provided.
  • Uniform color: burgundy scrubs.
  • Inpatient only.
  • Weekend shifts: 2 per month (every other weekend schedule).
  • Holidays may be required.
  • Shift: 8-hour day shifts, typically starting at 8:00 AM.

Required Qualifications

  • Active California Physical Therapist license.
  • Minimum 2 years of physical therapy experience.
  • Current BLS certification (AHA).
  • Able to provide 2 supervisory references from modality submissions: one within the last 12 months and one within the last 3 years.
  • Must not have been directly employed (full-time, part-time, or PRN) by any facility associated with the specified health system within the past year; PRN employment through outside firms is acceptable.
  • Valid government-issued ID matching legal name on application (no middle initials or names).

Additional Information

  • No prescreening; interviews conducted by the manager.
  • Coverage required for multiple stations/units labeled A, B, C, and D.
  • Candidates must be comfortable floating between skilled nursing and inpatient acute care settings.
