Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 1:30 PM – 12:00 AM
- Work Environment: Trauma level one for pediatric and adult patients
- Rotation Areas: Surgery, ports, pediatrics, and emergency room
Required Qualifications
- Valid Radiology Technologist license in Ohio
- Minimum of 1 year of Radiology Technologist experience within the last year
- ARRT certification required
- Valid Driver's License
- Experience as an X-Ray Technician
Health and Safety Requirements
- MMR: Positive titers within the last 15 years; booster protocol applies if low titer.
- Varicella: Positive titers within the last 15 years; booster protocol applies if low titer.
- Tdap: Vaccination within the last 10 years; no declinations accepted.
- Hepatitis B: Proof of full series, partial series, or signed declination form accepted; no childhood vaccines allowed.
- COVID-19 Vaccine: Preferred but not required; medical or religious exemptions accepted.
- Influenza Vaccine: Required during flu season (October–March) or mask-wearing and specific declination forms must be completed.
- Tuberculosis Testing: 2-step PPD or IGRA test with negative results within the last 6 months; if positive, clear chest X-ray and current TB questionnaire required.
- Fit Test: Completed within 1 year prior to start; updated annually.
- Physical Exam: Pre-employment physical exam within one year confirming ability to work without restrictions.
Background and Compliance
- Criminal Background Check: Within 6 months of start, including social security trace, county searches for last 7 years, national criminal sweep, and sex offender registry.
- Fingerprinting: Ohio residents must complete Ohio BCI&I fingerprint check and FCRA-compliant national database check; out-of-state residents require FBI fingerprint check.
- Employment Verification: Verification of employment within the last 5 years through third-party.
- Education Verification: Highest level of education verified through third-party; international education requires equivalency documentation.
- I-9 Attestation: Proof of eligibility to work in the U.S. required.
- OIG and Federal Exclusion Checks: Must be completed within 6 months prior to start.
- Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted.
- Skills and Competency: Skills checklist (clinical specific) completed within 1 year prior to start; competency exam and core competencies with minimum 80% score required within 1 year prior to start.
- Safety and Compliance Training: Completion of required safety modules prior to clearance; renewal annually.

- Two professional references from the last 2 years
- Completed job application form
- Employment history form confirming previous employment at the facility system (if applicable)
- Skills checklist (clinical specific)
- Non-expired photo ID (driver’s license or state ID)

- Badge photo required (headshot, shoulders up, plain background, JPEG format).
- Attestations are generally not accepted except for fingerprinting pending status with proper documentation.
- Some health requirements (e.g., boosters for MMR and Varicella) have specific timing and documentation protocols.
- Annual renewal requirements apply for fit testing, TB testing, drug screening (if break in assignment), and safety training modules.
