Registered Nurse (MS/Telemetry) - Travel/Contract
Location: Mayfield Heights, Ohio
Shift: Night (4 shifts per week, 12 hours each; 7:00 PM to 7:30 AM)
Contract Length: 5 weeks
Job Summary
- Registered Nurse position on Medical-Surgical/Telemetry unit.
- Travel/contract assignment.
Required Qualifications
- Valid Registered Nurse (RN) license in Ohio.
- Minimum of 1 year of Medical-Surgical/Telemetry nursing experience, with experience within the last year.
- Current BLS certification issued by the American Heart Association or American Red Cross.
- NIH Stroke Scale (NIHSS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Valid government-issued Driver’s License.
Certifications (Must be current and renewed if expiring within 30 days of start)
- BLS (Basic Life Support) – Required for all candidates.
- ACLS (Advanced Cardiac Life Support) – Required for Medical-Surgical/Telemetry.
- PALS (Pediatric Advanced Life Support) – Preferred but not required for this unit.
- NRP (Neonatal Resuscitation Program) – Not required for this specialty.
- Other certifications such as CNOR, CHEMO, TNCC are not required or only preferred for other specialties.
Background and Screening Requirements
- Comprehensive background check including 7-year criminal history, SSN trace, sex offender registry, OFAC, FACIS Level 3, and education verification.
- Monthly EPLS/GSA/SAM and HHS/OIG checks.
- FBI and BCI fingerprinting required within 12 months of start and updated annually; must be completed within 48 hours of contract signing. Facility will also conduct fingerprinting onsite.
- Drug screening: Standard 10-panel drug screen within 12 months of start, updated annually. Instant drug screens not accepted.
Health and Immunization Requirements
- Physical exam within 1 year of start confirming ability to safely perform job functions.
- Fit mask test (3M N95 1860 or 1870+) within 12 months of start, updated annually. Facial hair exemptions require approval.
- Immunizations/vaccinations required with official documentation:
- Tdap (Tetanus/Diphtheria/Pertussis) every 10 years (no declinations accepted).
- Measles (Rubeola), Mumps, Rubella – 2 MMR vaccines or positive titer; booster required if titers negative/equivocal.
- Hepatitis B – Titer, series, or declination with agency-approved exemptions.
- Varicella – 2 vaccines or positive titer; booster if titers negative/equivocal.
- Seasonal Flu vaccine required annually by November 30th; no declinations accepted (medical/religious exemptions require agency approval).
- Negative TB test (2-step PPD, QGold, or TSPOT) within 12 months or chest X-ray if past positive, with annual TB questionnaire if applicable.
- Snellen eye exam within 1 year of start.
Documentation and Compliance
- Employment application with at least 3 years prior work history including 2 years clinical practice in acute care and similar area.
- Two professional references from within 1 year of start (manager, supervisor, charge nurse, or director).
- Completion of facility-specific online onboarding modules prior to start (must be done on a computer using Google Chrome).
- Baseline competency exams (medication administration and ECG interpretation) required for all bedside RNs except OR RNs; passing scores required.
- Photo for badge (headshot, shoulders up, white background) required for certain tech roles but not specified for RN.
- Acknowledgement of pass/fail testing and orientation safety manual as part of onboarding.
Additional Notes
- Candidates can start with fingerprinting results pending if fingerprinting is completed prior to start.
- Physical exam and eye exam do not need to be updated annually if currently on assignment without a break; gaps longer than 30 days require updated exams.
- Agency-approved medical and religious exemptions for vaccines and mask fit testing must be submitted for facility review and approval.
- Flu vaccine exemptions require agency attestation and facility approval.
