Job Details
- Position: Registered Nurse (Operating Room)
- Location: Ontario, Oregon
- Employment Type: Long-Term Assignment (approximately 13 weeks)
- Shift: 06:45 AM – 5:15 PM
Required Qualifications
- Valid RN license in Oregon
- Minimum of 1 year of Operating Room (OR) experience within the last year
- BLS certification issued by the American Heart Association
- PALS certification
- ACLS certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Required Onboarding Documentation and Compliance
- License and certification verifications dated within 30 days of start.
- Fit test.
- 10-panel drug screen completed within 30 days of start (no THC).
- Specialty-specific competency testing completed within the last year.
- COVID-19 vaccination proof (including booster by September 2025), or approved declination/exemption (declination not accepted in CA or IL).
- MMR vaccination proof (2 doses or IgG titer).
- Hepatitis B vaccination series initiated prior to start.
- Influenza vaccine proof or declination (optional).
- 2-step PPD or IGRA test; if positive, chest x-ray per regional requirements.
- Varicella vaccination proof or IgG titer.
- Physical/health statement confirming fitness to work within the last year.
- Completion of professional forms including:
- Code of Conduct
- Conflict of Interest Disclosure
- Criminal Background Check (7 years, including SSN verification)
- Education Verification
- EPLS and OIG exclusion checks (within 30 days)
- Integrity and Audit Services Form (signed near start date)
- Joint Commission CORE Exam (within 1 year)
- National Sex Offender Registry check (within 30 days)
- System Access Form (completed and uploaded one week before start)
- Resume and skills checklist (within 1 year) and two professional references.
Additional Notes
- All documentation must be uploaded to the credentialing system prior to or at onboarding.
- Specialty competency and skills checklists are required for clinical contractors.
- Drug screen panel includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates.
