Location: Mequon, Wisconsin
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 06:00 AM – 06:30 PM

Job Description

• Every Other Week (EOW) and Every Other Holiday (EOH) schedule
• Minimum 2 years of experience as a Respiratory Therapist, including:

  • Critical care
  • Intubation
  • Arterial line insertion
• Prior travel experience required
• Experience with EPIC electronic health record system
• May be required to float onsite within skill set or to other sites within 40 miles

Required Qualifications

• Valid Registered Respiratory Therapist license in Wisconsin
• Minimum 2 years of Registered Respiratory Therapist experience, including experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association
• Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association
• Advanced Cardiovascular Life Support (ACLS) certification issued by the American Heart Association
• Neonatal Resuscitation Program (NRP) certification issued by the American Heart Association
• Valid driver’s license
• Experience in intubation
• Experience in arterial line insertion
• Critical care experience
• Experience with Epic electronic health record system

Required Documentation for Submission

• Professional resume/profile
• Two professional references
• Current license and primary source verification within 30 days prior to start
• Driver’s license/state ID (front and back)
• Ascension Resume (specific to this assignment)

Required Documentation for Onboarding (to be completed/uploaded prior to or at start)

• Signed assignment agreement (offer letter, initial confirmation, and extensions)
• Respiratory Therapy Contract Worker Onboarding Education Attestation (6 hours pre-hire training)
• BLS, PALS, ACLS, NRP certifications (AHA or equivalent)
• Ascension Core Competencies completion
• Ascension Badge Photo (forward-facing headshot with white or blue background, no filters)
• Immunization and health documentation as listed above
• Fit test documentation
• Ascension Criminal Background Check results (including Social Security Trace, OIG Search, National Sex Offender, and others) within 90 days of start
• EPLS check within 90 days of start
• Ascension Skills Checklist for specialty, updated annually
• Ascension 12 Panel Drug Screen results
• Education verification relevant to clinical work within 90 days of start
• Confirmation of assignment on agency letterhead with accurate start/end dates
• Employment verification for past seven years, updated annually

Additional Notes

• The Respiratory Therapy Contract Worker Form is completed onsite and uploaded by the facility; no need to submit prior to start
• Flu vaccine is optional; if declined during flu season (Oct 1 – Mar 31), mask wearing is required at all times
• Floating to other sites within 40 miles may be required based on staffing needs

This position requires a highly skilled respiratory therapist with critical care experience and prior travel assignments, ready to work a long-term contract with a structured onboarding and credentialing process.
