Location: Elmira, New York
Employment Type: Long-Term Assignment (13 weeks)
Shift: 7:00 AM – 3:30 PM
Call Requirements:

• One weekday call every two weeks
• Weekend call rotation

Job Description

• Must be competent in Orthopedics, DaVinci robotic surgery, Neurosurgery, General Surgery, and Gynecology
• Open to 8 or 10 hour shifts

Required Qualifications

• Valid RN license in New York
• Minimum 1 year of Operating Room (OR) experience, including experience within the last year
• ACLS certification
• BLS certification issued by American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Valid Driver's License
• Competency in Orthopedics, DaVinci robotic surgery, Neurosurgery, and Gynecology
• Experience in Orthopedics, Neurology, and OB/GYN specialties

Onboarding and Compliance Requirements

Skills and Competency

• Completion of unit-specific skills checklist, updated annually
• Core Competency Exam (Nursing Core Comp I, II, and III) with a minimum score of 80%, updated annually

Health and Immunizations

• COVID-19 vaccine (optional: proof of vaccine or agency declination)
• MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start
• Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start
• Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required
• Tdap: Within last 10 years or specific declination form required
• TB Test: One of the following required and must be current within 12 months:

  • Two PPD skin tests (one within 12 months, one within 3 months)
  • One Quantiferon/Spot test within 12 months
  • For past positive TB tests, chest x-ray within 12 months may be required
• Fit Test: Must be completed within last 12 months prior to start; on-site fit testing available if needed
• Physical/Health Statement: Within past 12 months, must confirm employee passed physical with no restrictions, including height, weight, vitals, and vision test

Background and Screening

• Agency background check within 30 days of start, including:

  • SSN address trace for all counties lived/worked in last 7 years
  • FACIS 3 check
  • County and national criminal searches
• 7 Instant Background Checks (manual upload required)
• Drug Screen: 9+ panel excluding marijuana (amphetamines, barbiturates, benzodiazepines, cocaine, opiates, oxycodone, phencyclidine, methadone, methaqualone, propoxyphene, nitrites)

Documentation

• Valid US driver’s license or state ID (cannot be expired)
• Color headshot photo (JPEG, no filters, no sunglasses)
• Resume covering all employment and gaps for last 7 years, with education details
• Two professional references:

  • One supervisor reference from last 12 months
  • One supervisor or peer reference from last 3 years
• Verification of most recent employer (agency verification accepted)
• Signed and dated job description
• Completion of various required forms and acknowledgements found in the document library, including:

  • Health personal information form
  • Code of conduct
  • Confirmation of assignment form
  • Self-study packet and quiz
  • Sexual harassment acknowledgement
  • Parking registration form
  • AH Drug Tobacco Acknowledgement
  • TB Questionnaire Form (specific to facility)
  • Education mission statement (for initial assignments)

Additional Notes

• Must maintain all certifications and immunizations current throughout the assignment
• Fit testing includes specific approved mask models; traveler must notify if fit test is needed on-site
• Physical must be performed by MD, DO, NP, PA, or DC and include specific clinical details
• Call requirements include weekday and weekend rotations as specified
