Ultrasound Technician - Iowa, Dubuque
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Shift: 2nd shift (3:00 PM - 1:30 AM) or 11:00 AM - 9:30 PM
- Schedule: Occasional call coverage and help covering hours every 6th weekend; holidays as available
- Contract Length: 13 weeks
Required Qualifications
- Minimum of 1 year of experience as an Ultrasound Technologist, with experience within the last year
- Current and valid license/certification for Ultrasound Technician, with verification dated within 30 days of start
- BLS certification issued by the American Heart Association
- Specialty-specific competency testing completed within the past year
- Fit test
- 10-panel drug screen completed within 30 days of start (no THC)
- COVID-19 vaccine proof (including booster by Sept 2025), declination or exemption accepted except in CA or IL
- MMR: proof of 2 vaccinations or IgG titer lab report
- Hepatitis B: must have started vaccination series prior to start (titers not accepted)
- Influenza vaccine proof or declination (optional)
- 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella: proof of 1 vaccination or IgG titer lab report
- Physical/Health statement from exam within last year confirming fitness to work
- Current, non-expired Tdap vaccine (declinations accepted if no pediatric interaction)
- Federal, nationwide, and county criminal background check from last 7 years, including SSN verification, dated within 30 days of start
- Education verification of highest academic degree or diploma/GED
- EPLS (Excluded Parties List System) and OIG (Office of Inspector General) checks dated within 30 days
- NSO (National Sex Offender) check dated within 30 days
- Child and Adult Dependent Abuse Training (Iowa only)
- Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check within 30 days prior to start
- Conflict of Interest Disclosure Form and Code of Conduct Form signed and on file
- Confidentiality and Information System Security Agreement signed and on file
- Integrity and Audit Services Form signed shortly before start
- Non-Employee Upload Form completed and uploaded
- System Access Form completed and uploaded one week before start
- Resume and skills checklist within the past year
- CORE exam and specialty-specific competency testing within the past year
