Schedule
- Shift: Day (4x10 hours), 7:00 AM to 5:30 PM
- Hours per week: 40
- No weekend shifts
- Floating within facility/scope of practice as needed
- Maximum Return to Office (RTO) time: 7 days
Caseload and Productivity
- Average daily census: 115 patients
- Typical caseload: 6 visits per day
- Full-time productivity: approximately 25 units per week
Travel and Mileage
- Mileage reimbursement paid at IRS standard rate
- Coverage area includes multiple counties with occasional travel to sister office counties
- Average daily mileage: 50-100 miles
Orientation and Compliance
- Full compliance with credentialing and onboarding required to start on time; delays in compliance may result in a 2-week start delay due to bi-weekly orientation scheduling
- Number of non-billable orientation hours depends on contract length (e.g., 8 hours for 8-week assignment)
Holidays
- If one major holiday falls during assignment, traveler is expected to work it
- If two major holidays fall during assignment, traveler is expected to work both
- If three major holidays fall during assignment, traveler is expected to work two of them
Major holidays include:
Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, New Year's Eve/New Year's Day
Required Documentation and Compliance Items
- Valid photo ID (Driver’s License or State ID)
- Background check completed within 30 days prior to start (including education verification)
- Education verification of related degree (can be pending at start with proof it is in process)
- Physical exam within 12 months prior to start
- Negative 10-panel drug screen within 60 days prior to start (including amphetamines, benzodiazepines, barbiturates, cocaine, opiates, methadone, PCP, propoxyphene, THC)
- Immunizations and titers:
- TB test (PPD, QGold, or TSpot) within 12 months or negative chest x-ray if prior positive
- TB questionnaire (updated annually)
- MMR (2 doses or positive titer)
- Varicella (2 doses or positive titer)
- Hepatitis B surface antibody titer or signed declination
- Flu vaccine or signed declination (required during flu season)
- Valid proof of current automobile liability insurance
- Passport-style photo with solid background for badge
- Review and sign facility’s Code of Conduct and Dress/Professional Appearance Policy
- Completion of HCHB training and passing associated assessment (if applicable)
Additional Requirements
- Skills checklist updated within 12 months
- Verification of most recent work history (can be pending at start with proof it is in process)
- Two clinical references required
- Color blindness test required (with alternative assessment if needed)
Requirements/Qualifications
- Valid Occupational Therapist license in Georgia
- Minimum 1 year of Occupational Therapist experience, including experience within the last year
- Current Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
- Experience in a Home Health setting
- Experience with Epic electronic health record system
