  • Active RN license
  • BLS certification
  • ACLS certification

  • Epic experience
  • Completion of the following onboarding items:

    • Rhythm interpretation exam
    • Driver’s license or state ID
    • Resume
    • 2 professional references
    • Job description acknowledgment
    • Employment verification
    • Nursys verification
    • Pharmacology exam
    • TB questionnaire form
    • Drug screen
    • Background check
    • Physical/health statement
    • Fit test
    • MMR documentation
    • Varicella documentation
    • Hepatitis B documentation
    • Tdap documentation
    • COVID-19 vaccine documentation or declination
    • Color headshot
    • Drug/tobacco acknowledgement
    • 7 instant background checks

Notes

  • BLS and ACLS must not expire while on assignment.
  • NIHSS must not expire while on assignment.
  • Fit test must be completed prior to start.
  • State license verification must be completed within 30 days prior to start.
  • Nursys must be completed within 30 days prior to start.
  • The physical/health statement must be within the past 12 months.
  • The TB test is an annual requirement.
  • The job description states every other weekend and every other holiday coverage.
