Case Manager
Job Overview
- Travel/contract role in Chesapeake, Virginia
- 13-week assignment
- Day shift: 5x8 hours, 8:00 AM–4:30 PM
- Weekdays, with some weekends and holidays
- No overtime without supervisor approval
- Casual business attire
Required Qualifications
- Valid RN license in Virginia
- Associate of Science in Nursing / Associate Degree in Nursing
- Acute Care Case Management (5 years of experience)
- Acute Care Case Management experience within the last year
- One completed travel assignment
- BLS - Basic Life Support issued by American Heart Association
- Experience working with Adult patients
- Experience in an Acute Care setting
- Discharge planning experience
- Experience with Epic
Preferred Qualifications
- CCM or ACM
Work Setting
- Discharge planning on an ortho/neuro surgical unit
- Fast-paced unit with multiple daily admissions and discharges
