Home Care Physical Therapist
Job Overview
- Location: Barre, Vermont
- Employment type: Contract
- Contract length: 13 weeks
- Schedule: Tuesday–Saturday, 8:00 AM–5:00 PM
- Hours per week: 36
- Setting: Home health / hospice
- Patient care model: One-on-one therapy in patients’ homes
Required Qualifications
- Valid Physical Therapist license in Vermont
- 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
- Experience in a Home Health setting
- Experience with OASIS
Preferred Qualifications
- McKesson/Netsmart EMR experience
Key Responsibilities
- Assess patient condition and develop individualized plans of care
- Collaborate with the interdisciplinary team to meet patient needs
- Perform standard therapy measurement techniques, including range of motion, strength, and balance tests
- Use therapeutic modalities to maximize care provided to clients
Scheduling Note
- No requested time off is preferred
