Physical Therapist

Overview

• Contract role in Sunnyvale, California
• Home health setting
• Day shift, 8:00 AM–5:30 PM
• 40 hours per week
• Schedule may be flexible based on patient availability
• One weekend day per month

Required Qualifications

• Valid Physical Therapist license in California
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS
• Experience in a(n) Home Health setting

Role Highlights

• Evaluate and treat patients using prescribed physical therapy programs
• Restore function and help prevent or minimize disability after disease or injury
• May supervise Physical Therapy Assistants
• May complete initial admission/evaluation visits and report OASIS scores within required timelines
• Document observations, clinical findings, interventions, goals, and discharge plans
